• Integration Health provides integrative healthcare services, which may include primary care, preventive care, therapeutic ketamine services, telehealth consultations, IV nutrient therapy, weight management, and other healthcare services within the scope of practice of the treating provider.

We may use and disclose information necessary to obtain payment for services rendered, verify insurance benefits, process claims, collect

payment, or conduct billing activities.

 

Healthcare Operations

We may use and disclose your information for practice operations, quality improvement, staff training, licensing requirements, accreditation activities, and business management functions.

 

Appointment Reminders and Communications

We may contact you by phone, voicemail, email, secure messaging, or text message regarding appointments, treatment follow-up, scheduling, billing matters, or other healthcare-related communications.

 

Business Associates

We may share information with trusted vendors and service providers who perform services on our behalf and who are legally required to safeguard your information.

Electronic Communications and Patient Portal

Integration Health utilizes secure electronic systems, including the Osmind patient portal, for appointment scheduling, intake forms, clinical questionnaires, treatment planning, billing communications, and secure messaging.

 

While reasonable safeguards are used to protect electronic communications, no method of electronic transmission can be guaranteed to be completely secure. Patients are encouraged to use the secure patient portal whenever possible for healthcare-related communications.

 

By providing your email address, mobile phone number, or other electronic contact information, you consent to receiving healthcare-related communications from Integration Health through secure electronic means as permitted by law.

 

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Integration Health or with the U.S. Department of Health and

Human Services.

 

You will not be penalized or retaliated against for filing a complaint.
